ASSESSMENT → REVIEW → ONGOING SUPPORT
Care is continuously reviewed and adapted to ensure the right level of support at the right time.
Assessment Process
Assessment considers physical health, functional ability, cognition, emotional wellbeing, environment, and support networks. Nursing input informs clinical oversight where required.
Outcome Setting
Outcomes are agreed with individuals and families and reviewed regularly, focusing on recovery, stability, independence, or long-term support.
Step-Down & Transition Planning
Transitions are planned from the outset and may include step-down from complex care, supported living, or long-term arrangements.
